Replacing a Lost or Stolen Passport
To prevent identity theft, please report any lost or stolen passport as quickly as possible. To do so, you must complete Form DS-64, available on-line (see below) or at the Embassy. Fill in as much of the form as possible and answer all the other quesitons in detail.
The information you provide will be entered into the Consular Lost/Stolen Passport Sytem. Passports reported lost or stolen are invalidated and can no longer be used for travel. If you recover the passport after you have reported it lost or stolen, please submit it to the Consular Section, where it will be cancelled and returned to you. Once a passport is reported lost or stolen, it cannot be re-validated.
To replace a lost or stolen passport, you must appear in person at the Embassy and bring the following items:
- Form DS-64, Statement Regarding a Lost or Stolen Passport, filled as completely as possible.
- Completed, but not signed, Form DS-11 -- DO NOT SIGN until instructed to by a consular officr.
- A police report showing your previous passport as being one of the items lost or stolen
- Two full-face 2 inch by 2 inch color photos taken within the last six months. The photos must meet these specifications.
- Proof of U.S. citizenship, if possible, such as an original birth certificate from the U.S., a Consular Report of Birth Abroad, or a Naturalization Certificate.
- Cash fee of $100 or equivalent in F CFA.

A full validity passport can usually be received witin 10 days. For emergency travel only, a passport can usually be issued the same day.
Any valid visas that are needed for travel that were in the lost or stolen passport are the reponsibility of the bearer. The Consular Section is not able to assist you in obtaining foreign visas.
