Passport Information
How to Apply for a U.S. Passport at the U.S. Embassy in Bamako
Adult Passport Renewal (Age 16 and Above)
If you are replacing a 10-year validity passport that was issued within the last 12 years and you are able to present this passport, you can apply for a new passport using Form DS-11. You do not need to appear personally at the U.S. and can send the completed form and original passport to the U.S. Embassy with someone you trust. Please do not send the application and passport by mail.
Required Items:
- Completed and signed DS-82.
- Two full-face 2 inch by 2 inch color photograph taken within the last six months with a white or light background.
- Original U.S. passport.
- Cash fee of $75 or equivalent in CFA.
If you are applying for your first adult passport; are replacing a lost, stolen, or mutilated passport; or are not able to present a 10 year validity passport that was issued within the last 12 years; you must appear in person to apply for a new U.S. passport.
Required Items:
- Completed, but not signed, DS-11 -- DO NOT SIGN until instructed to by a consular officer.
- Two full-face 2 inch by 2 inch color photograph taken within the last six months with a white or light background.
- Proof of citizenship, such as a previous U.S. passport or original birth certificate.
- Proof of identity, such as a photo i.d.
- Cash fee of $100 or equivalent in CFA ($75 issuance fee, $25 execution fee).
Children Under the Age of 16
For children under age 16, both parents and the child must personally appear at the U.S. Embassy.
Required items:
- Completed, but not signed, Form DS-11 -- DO NOT SIGN until instructed to by a consular officer.
- Two full-face 2 inch by 2 inch color photograph taken within the last six months with a white or light background.
- Evidence of the child's relationship to the parents/guardians, such as an original birth certificate or consular report of birth abroad. Previous U.S. passports are not acceptable as proof of relationship to the applying parent(s)/guardian(s).
- Proof of identity for both parents.
- Proof of child's citizenship, such as a previous U.S. passport or original birth certificate.
- Proof of child's identity, such as a photo i.d.
- Cash fee of $85 or equivalent in CFA ($60 issuance fee, $25 execution fee).
If a parent cannot personally appear and sign the Form DS-11, then the other parent must submit:
- A completed an notarized Form DS-3053 (Statement of Consent: Issuance of a Passport to a Minor Under Age 14)
AND - The absent parent's identification information. A copy of his/her ID may be used for this purpose.
A parent who has sole authority to apply for a passport must present proof, such as a birth certificate listing only the applying parent; a court order granting sole custody to the applying parent (unless child's travel is restricted by that order); a court order specifically permitting applying parent's or guardian's travel with the child; or a death certificate of non-applying parent.
Reporting a Lost or Stolen Passport
To prevent identity theft, please report any lost or stolen passports as quickly as possible. To do so, you must complete Form DS-64 (Statement Regarding a Lost of Stolen Passport), which is available from the Consular Section or at the following website: http://travel.state.gov/passport/forms/ds64/ds64_845.html Fill in as much of the passport information as you can and answer all the other questions in detail. Sign and submit the form to the Consular Section with a copy of the police report, if possible.
The information you provide on the DS-64 (Statement Regarding a Lost or Stolen Passport) will be entered in our Consular Lost/Stolen Passport System. Passports reported lost or stolen are invalidated and can no longer be used for travel. If you recover the passport after you have reported it lost or stolen, please submit it to the Consular Section, which will cancel it and return it to you. Once a passport is reported lost or stolen, it cannot be re-validated.
Passport Pages
If you would like to insert new passport pages in your passport, please complete and sign Form DS-19 and send it to the Consular Section with your original passport. If possible, we will insert the new pages while you (or the person who brought your passport) wait.
Consular Section Hours
The Consular Section is open for routine American citizen services on Thursdays from 8h00 to 15h00 and on other days by appointment. The Consular Section is closed on all U.S. federal and Malian holidays. In cases of Emergency, please contact the U.S. Embassy at (223) 270-2300.
