Extra Pages
If you would like to insert additional pages into your pasport, please complete and sign Form DS-4085 and bring or send it to the Consular Section with your original passport. We are usually able to insert the new pages the same day.
NOTE:
Department of State regulations state that only passports which are well cared for, with very little wear and tear, may have visa pages added. This determination is made by the consular officer. If your passport is in poor condition, you may be asked to replace your passport.
